Director of Clinical Services (RN) Home Health
We are fixing US healthcare by building an AI-native physical care platform, starting with home health.
Home health is a $140B industry with less than $10B in EBITDA — 40% of revenue is spent on pure administrative waste. We automate this work with AI, creating a fundamentally different cost structure compared to incumbents. This lets us rapidly take market share by serving the 30% of patients who go untreated today.

The Opportunity
We are looking for a Director of Clinical Services who can lead clinical execution with high standards and high intensity.
This is not a passive “clinical support” role. You will own day-to-day clinical coordination and quality, ensuring that:
Patients receive the right care at the right time
Clinicians are supported, coached, and held to clear expectations
Changes in condition are identified early, escalated appropriately, and documented thoroughly
Care plans, orders, and clinical documentation are accurate, timely, and compliant
You will partner closely with operations and scheduling to keep care delivery on track, and you will be a key leader in how we scale safely.
What You’ll Do
1. Own clinical execution for your branch
Monitor clinical status across active patients and ensure timely follow-up when a patient’s condition changes
Coordinate next steps after clinical events (new symptoms, missed visits, safety concerns, hospitalizations, discharges)
Ensure care plans are clear and actionable for field clinicians, with appropriate coordination of orders and plan updates
Maintain high standards for clinical documentation quality and timeliness
2. Lead and develop field clinicians
Coach clinicians on clinical reasoning, visit planning, patient education, and documentation quality
Set and reinforce expectations for reliability, professionalism, and patient experience
Address performance issues quickly and fairly, including structured improvement plans when needed
Support onboarding and skill development to raise clinical consistency across the team
3. Drive quality, safety, and compliance through systems
Identify recurring clinical and documentation issues and implement process improvements
Support quality assurance activities by ensuring clinical work meets standards before downstream review
4. Be an escalation leader
Serve as a calm, decisive escalation point for clinical questions and patient risk
Know when to intervene directly and when to empower field staff with guidance
Coordinate with leadership on high-risk cases, incidents, and sensitive patient situations
Who You Are
A high-accountability clinical leader who loves building systems and developing people. You bring urgency, clarity, and standards to complex clinical operations.
Must-haves
Ability to coach, mentor, and train clinical staff
Strong clinical judgment and a structured approach to prioritization and escalation
A strong sense of personal accountability. You must take ownership of clinical outcomes through your clinical staff.
Ability to communicate clearly across clinical and operational teams
How you work
Owner mindset: You prefer accountability over recommendations
Bias to action: You solve problems quickly while staying safe and compliant
Systems thinker: You standardize and improve workflows instead of relying on heroics
High standards: You hold the line on quality, documentation, and patient experience
Low ego: You are willing to do hands-on work and teach others to do it well
Nice-to-haves
Home health experience
Experience building or improving clinical workflows at scale
Experience partnering closely with operations teams to prevent service failures
Experience
Minimum of one (1) year of recent clinical experience as a Registered Nurse
At least one (1) year of experience in a Home Health setting is required
Previous leadership or supervisory experience in home health or another clinical care environment is strongly preferred
Thorough understanding of home health operations, including coordination of care, compliance with Medicare Conditions of Participation (CoPs), and management of OASIS documentation
Proficiency with Homecare Homebase (EHRs) and experience managing patient documentation and workflows in Homecare Homebase
Demonstrated ability to lead interdisciplinary teams, oversee patient care plans, and support clinical staff performance
Experience with utilization review, quality assurance, and performance improvement in a home health context is a plus
Working Conditions
Primarily office-based with occasional field responsibilities
May require local travel and patient home visits as needed
Must be able to manage multiple tasks and work in a fast-paced environment

Must be able to lift up to 50 lbs and support patients as needed
