Pay Range

$38.20 - $57.30

We’re Looking For: 

  • BSN required. Masters preferred.
  • Registered Nurse License issued by the state in which the Team Member practices.
  • Two years' clinical nursing experience required in health care.
  • Experience includes case management/discharge planning in one of the following settings: Acute care, Home care, LTC care, Physician Office, or Managed Care company.
  • Appropriate professional certification required within 3 years of hire date for professional certification per departmental protocol.
  • Clinical competence in disease management and case management principles.
  • Must possess excellent interpersonal communication and negotiation skills, problem-solving skills, strong organizational and time management skills, and the ability to work independently and as a member of the care team.
  • Requires demonstrated knowledge and proficiency in appropriate tools.
  • BLS required per policy guidelines.

What You’ll Do: 

Schedule: Full time, 1st shift,  Monday- Friday business hours, this position does require a weekend and holiday rotation

  • Apply the nursing process to coordinate care, optimize patient outcomes, and ensure effective resource use.
  • Collaborate with patients, providers, and the community to maintain continuity of care and develop care plans across settings.
  • Conduct comprehensive assessments, align services with patient needs, and manage accurate documentation and case records.
  • Maintain knowledge of reimbursement requirements and demonstrate leadership by adhering to professional nursing standards and evaluating personal practice.

Physical Requirements:

  • Must lift up to 10 lbs. continuously, up to 20 lbs. frequently, and up to 50 lbs. occasionally.
  • Manual dexterity required for operation computer and calculator.
  • Visual acuity required for facilitating review of written documents/computer screens, medical records, and to record information accurately.
  • Clear verbal communications and hearing acuity required for receiving instructions and converse on standard telephone.
  • Functional speech and hearing to allow for effective communication of instructions and conversation over the telephone.
  • Exposed to normal office environment; including usual hazards related to operating electrical equipment.
  • Operates all equipment necessary to perform the job.
  • Must be able to sit up to approximately 50 percent of the workday; stand and walk for the equivalent of several blocks at a time.
