Position Overview

We are seeking a dedicated Advanced Practice Provider (APP) to join our Nephrology team in a full-time role. This position offers a unique opportunity to work across two hospital systems, providing a diverse and well-rounded clinical experience.

The APP will be responsible for inpatient rounding, outpatient hemodialysis management, and clinic-based care. The role focuses on the ongoing management of patients with chronic kidney disease (CKD) and hypertension, while supporting continuity of care across multiple settings.

This opportunity is ideal for a clinician looking to broaden their scope of practice while building meaningful, long-term patient relationships.

Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Call: Rotating after-hours call; no weekend requirements

Primary Responsibilities

Clinical Care & Patient Assessment

  • Conduct comprehensive patient evaluations, including history, physical exams, and review of systems
  • Diagnose and treat acute conditions and manage chronic illnesses
  • Develop differential diagnoses and implement appropriate treatment plans
  • Order, interpret, and act on diagnostic tests
  • Monitor disease progression and treatment response
  • Determine need for referrals, hospitalization, or physician intervention
  • Ensure appropriate follow-up and continuity of care
  • Adhere to infection prevention and control protocols

Patient Education

  • Educate patients and families on disease processes, treatment plans, and preventive care
  • Promote healthy lifestyle behaviors and self-care practices
  • Assess and adapt education to patient learning needs and readiness
  • Utilize educational materials and document outcomes

Documentation

  • Maintain accurate, timely, and compliant medical records
  • Clearly document patient encounters and care plans
  • Update problem lists and medication records regularly
  • Document patient communications, including phone calls and care coordination
  • Ensure documentation supports appropriate coding and billing

Professional Development & Clinical Competency

  • Maintain and enhance clinical knowledge through continuing education
  • Participate in required training and competency programs
  • Apply evidence-based practices in patient care

Collaboration & Leadership

  • Work collaboratively with physicians and multidisciplinary team members
  • Support clinical staff with guidance, education, and triage when needed
  • Contribute to improving patient flow and care delivery processes
  • Serve as a role model for patient care and professional conduct

Prescriptive Authority & Protocol Compliance

  • Prescribe medications in accordance with state regulations and practice agreements
  • Adhere to established clinical and drug protocols
  • Maintain current and compliant collaborative agreements

Quality, Safety & Compliance

  • Provide patient-centered care that respects dignity and confidentiality
  • Follow all organizational safety and infection control policies
  • Accurately code services and procedures
  • Participate in quality improvement and performance initiatives

Additional Responsibilities

  • Participate in rotating after-hours call coverage
  • Support efficient and cost-effective care delivery
  • Engage in professional development and organizational initiatives

Qualifications

  • Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
  • Experience in nephrology or related clinical setting preferred
  • Strong clinical, communication, and organizational skills
