Job Overview:
Riverview Psychiatric Center is currently recruiting for an Intensive Case Manager to work in their social work department. This position provides professional services work in the area of intensive case management for high-risk mental health clients within Riverview Psychiatric Center and during their transition into the community. As an Intensive Case Manager, you will act as a single identifiable support in a comprehensive and individualized approach to transition clients from the hospital into the community ensuring basic living needs are met and assisting clients in gaining access to medical and psychiatric requirements, rehabilitation, social, peer, educational, psychological support services. You will directly monitor client integration progress and overall service delivery system. Applicants must demonstrate an understanding of/worked within a comprehensive community mental health system.
Minimum Qualifications:
A Bachelor’s Degree in one of the Social or Behavioral Sciences and two (2) years’ experience in professional community mental health work. A Master’s Degree in Social Work may be substituted for two (2) years’ experience.
Licensing Requirements:
You must have the ability to obtain a Social Worker (LSW) license as issued by the Maine State Board of Social Worker Licensure.
