Physician - IDD Primary Care - Grand Rapids, Michigan
Corewell Health in Grand Rapids, Michigan is seeking a compassionate, mission-driven Family Medicine Physician to join our innovative Intellectual & Developmental Disabilities (IDD) Integrated Practice.
This is a unique opportunity to help build and shape a specialized, interdisciplinary care model designed to meet the complex needs of patients age 16 years and up with moderate to severe IDD. The primary care physician in this practice will play a key leadership role in advancing care delivery, improving outcomes, and ensuring a sustainable, patient-centered model.
Position Highlights
Innovative care model that focuses on the IDD population providing whole-person care
Balanced role with leadership responsibilities that will be approximately 40% traditional primary care, 40% primary care for the IDD population, and 20% administrative time
Extended visit times for the IDD population with longer, more meaningful patient and family interactions
Smaller IDD panel size
Multidisciplinary clinic with a behavioral analyst (BCBA), social work, nurse navigator and psychiatry support
Purpose-build clinic with provider, patient and family needs at the center
Select Responsibilities
Provide comprehensive primary care services for patients age 16+ with moderate to severe IDD
Collaborate with interdisciplinary team members to coordinate complex care needs
Support development and growth of the practice model, including care pathways and best practices
Build strong relationships with internal and external stakeholders
Participate in education and advocacy efforts for IDD care across the system and community
Contribute to program innovation and quality improvement initiatives
Qualifications
MD or DO, Board Certified or Board Eligible in Family Medicine
Interest in, or passion for caring for individuals with IDD
Fellowship training in IDD preferred, but not required
Open to new graduates/residents with a strong interest in this population
