About Corewell Health South – Lakeland Hospitals 

The health system comprises three hospitals, an outpatient surgery center, a regional cancer center, rehabilitation centers, two long-term care residences, home care and hospice services, and 34 affiliate physician practice locations. We provide both inpatient and outpatient medical services throughout Southwest Michigan.

 

Scope of Work 

Assesses, monitors, detects, plans and takes appropriate action to prevent potential physiological complications associated with wound, ostomy or continence. Provides information and instruction to patients and their caregivers. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community. Participates in projects related to area of Wound, Ostomy and Continence Nursing and developing and evaluating educational materials and programs and policies.

Essential Functions 

  • Assesses and monitors the overall physical needs of the patients/family in regard to ostomy, wound and continence. Evaluates each individual patient’s response to treatment.
  • Assists in developing/maintaining an individual plan of care with the patient, consistent with changing needs and goals reflecting full scope of practice.
  • Collaborates in discharge planning for patient at time of patient contact, including families, caregivers as appropriate. May facilitate a smooth transition to the next point of service/home.
  • Assesses the educational needs of a select group of patients, caregivers, and staff. Develops and implements appropriate teaching strategies and makes appropriate referrals for patients providing counsel to enable them to adjust to living with a specific medical condition. Documents nursing care/teaching consistent with documentation guidelines. Ensures that required documentation for activities within area of responsibility is performed in an accurate and timely manner.
  • Collaborates with nurses, providers, and other departments to develop, implement, and evaluate programs and services that support patient care delivery in Wound, Ostomy and Continence Nursing. This includes effective utilization of workforce, equipment, and supplies. Develops interdisciplinary relationships to support improvements in care delivery and patient, staff and provider satisfaction.
  • Collaborates with nursing quality department to develop and maintain quality monitoring system to continuously monitor and measure the quality, safety, and outcomes of nursing services for assigned area. Develops and implements unit quality indicators to evaluate performance and the level of practice for Wound, Ostomy and Continence nursing as well as develops/implements improvement plans to correct variances.
  • Participates in the development and review of policies, procedures and guidelines related to wound, ostomy and continence based on evidence-based practice, research findings and Corewell Health quality outcomes.
  • Encourages an environment supportive of nursing research, promoting an evidenced-based, clinically effective, nurse-sensitive patient outcomes for Wound, Ostomy and Continence Nursing.

Qualifications 

Required

  • Bachelors in Nursing
  • Experience teaching patients (pediatric and/or adult), families, and other health professionals 
  • License - MI Registered Nurse (RN)  
  • Successful completion of a competency-based orientation program
  • CRT-Wound Care, Certified (WCC) - NAWCO National Alliance of Wound Care and Ostomy within 2 years Or
  • MAG_CRT-WOUND_OSTOMY_NURSE-WOCNCB within 2 years
  • AHA or ARC Basic Life Support (BLS) within 90 days
