About COREWELL HEALTH VNA
Our Visiting Nurses Association (VNA) is the Home Care Team serving West Michigan. We are seeking a licensed Occupational Therapist to join our growing team supporting patients across West Michigan (specifically Big Rapids, Reed City and surrounding areas).
This role offers a high level of autonomy, allowing clinicians to practice at the top of their license while delivering care in patients’ homes. Home health occupational therapy provides a unique opportunity to help patients regain independence with daily activities and safely function in the environment where they live and thrive.
Our therapists maintain manageable caseloads, typically seeing 5–7 patients per day, with thoughtfully designed geographic territories that minimize travel and support work-life balance. As part of Corewell Health West, our VNA team is committed to delivering exceptional, patient-centered care across Ottawa County and surrounding communities.
Scope of Work
In collaboration with an interdisciplinary team, provides therapeutic services including evaluation, treatment, and education. Monitors the treatment activities of COTA’s and aides. Completes evaluations of assigned patients in accordance with the established standards of care and practice. Plans and provides treatment to assigned patients as appropriate in accordance with established standards of care and practice. Completes all documentation of services per department standards. Attends all required safety training programs and can describe his or her responsibilities related to general safety, department/service safety, and specific job related hazards. Follows the hospital exposure control plans/bloodborne and airborne pathogens. Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment. Promotes effective working relations and works effectively as part of a department/unit team inter- and intra-departmentally to facilitate the department’s/unit’s ability to meet its goals and objectives. Maintains updated license and submits evidence of status to supervisor/manager within two weeks of receipt of new or updated license.
STANDARD QUALIFICATIONS

· Graduate from an accredited OT school.
· Successful completion of clinical internship.
· Previous clinical experience/rehabilitation experience preferred.
· Current State OT license.
· BLS certification.
1 year or more experience working in Home Care, VNA (Preferred)
