Eliot is seeking a Behavioral Health Services Manager to support the Collaborative Care Model (CoCM). Eliot’s CoCM is an integrated care approach that coordinates behavioral health, substance use, and medical services across primary care and community-based settings to improve access, continuity, and outcomes. The Behavioral Health Services Manager functions as part of the primary care team and supports shared treatment planning, provides evidence-based interventions, assists with care coordination, and monitors patient progress.
The ideal candidate demonstrates a strong commitment to mental health recovery, substance use disorder treatment, integrated care coordination, and overall patient wellness. The role requires comfort with providing community-based support, including traveling as needed to deliver services where patients are located.
Responsibilities:
- Functions as part of the primary care team and works under the direction and clinical supervision of the Primary Care Provider (PCP).
- Develops and implements Individualized Care Plans in collaboration with the patient, family, and collaterals.
- Provides evidence-based behavioral interventions including behavioral activation, problem-solving treatment, and motivational interviewing.
- Provides side-by-side interventions focused on problem-solving, skills training, behavioral modeling, and encouragement as outlined in each patient's treatment plan.
- Systematically tracks treatment response and monitors patients - in person or by telephone - for changes in clinical symptoms, treatment side effects, or complications.
- Ensures patient safety through crisis intervention as needed.
- Promotes community integration by supporting the use of community resources, encouraging community membership, and reducing stigma through education.
- Provides or coordinates transportation and mobility resources to enable full participation in medical and community appointments.
Qualifications:
- Master’s degree in human services or Bachelor’s degree in related field with at least five years experience.
- Experience in behavioral health care management, case management, or a related clinical role.
- Must have a valid driver’s license and reliable transportation for providing services in the community.
Schedule: Monday-Friday, 9:00 AM-5:00 PM
This is a community outreach position serving clients throughout the North Shore region, including Lynn, Peabody, Salem, Stoneham, and Melrose, MA. Candidates must be willing and able to travel throughout the service area and provide coverage across multiple office locations as needed.
