Be a Bridge to Better Health – Join Us as a Community Health Worker
Schedule: Monday - Friday 8:00am-5:00pm. 40 hours weekly.
As a Community Health Worker, you’ll serve as a vital link between individuals and the services they need to thrive—connecting people to healthcare, social services, and community resources. Through education, advocacy, and ongoing support, you’ll help individuals take control of their health and navigate systems that may otherwise feel overwhelming. This role directly supports our mission to improve lives and promote wellness in the communities we serve.

- Serve as a liaison between patients, providers, and community resources
- Assist patients with navigating healthcare, insurance, and social service systems
- Provide culturally appropriate education on health and wellness topics
- Support medication adherence, follow-up care, and self-management goals
- Help patients overcome barriers by arranging transportation, coordinating care, and advocating for needed services
- Document interactions and participate actively in the primary care team
- Build trust and promote health literacy within the community
- Support patients during post-hospitalization or after missed appointments
- Implement and document PRAPARE assessments as part of care planning
High School/GED required
- Bachelor’s degree preferred
LICENSURE/CERTIFICATION
Successfully complete the accredited Community Health Worker program
