Job Description:
The Ambulatory Care Pharmacy Specialist provides pharmaceutical care services in assigned clinic or hospital areas. Responsibilities include designing, implementing, monitoring, evaluating, and modifying patient pharmacotherapies to ensure that medication is indicated, effective, and safe. The specialist will participate in collaborative practice and/or protocol-driven practice as appropriate, and complete documentation for pharmacist activities. The specialist will participate in and support research activities of the department and the clinic areas served as well as support the continuous improvement, patient safety, and accreditation programs of the department and institution. Additional responsibilities and teaching experiences will vary as well as involvement in special projects and committees. The individual must display strong initiative, leadership, independence, collaborative, and communication skills.Education
