Sturdy Health Medical Group is currently seeking a dynamic and passionate Ambulatory Pharmacist Specialist with a focus on Healthcare Quality outcomes to join our progressive medical group. The primary purpose of this role is to develop and lead pharmacy collaborative practice models in chronic disease management aimed at advancing quality and population health clinical outcomes. The ideal candidate for this role demonstrates high standards of professionalism and a genuine commitment to improving quality of patient care. The candidate is an excellent communicator, comfortable interacting with a diverse range of people and is able to explain complex pharmaceutical information in a way that patients and other healthcare providers can understand.Required Qualifications and Skills:
- Minimum of three years of experience in a clinical practice setting with demonstrated skills (or residency equivalence)
- Advanced knowledge and understanding of chronic disease management, population health principles, practice models, and the healthcare landscape
- Experience in quality improvement strategies
- Demonstrated ability to work on a team and coordinate collaborative efforts

- Excellent interpersonal, verbal, and written communication skills
- Strong analytical, problem-solving, and decision-making abilities
Essential Job Functions:
- Direct patient care in the ambulatory environment including educating and medication management of patients with chronic conditions
- Develop, implement, and manage pharmacist-provider collaborative practice models, strategies, and initiatives aimed at improving quality of care and clinical outcomes
- Collaborate with multidisciplinary teams to design and implement population health programs focusing on chronic disease management, prevention, and wellness
- Education of healthcare providers and clinical staff on medication therapy and pharmaceutical care principles
- Serve as a preceptor for trainee pharmacists
- Lead medication policy and procedure development for ambulatory care practices
- Facilitate care transitions after emergency department discharges for at risk patients
- Participate in the formulation of population health policies, strategies, and guidelines
- Facilitate medication management services to optimize patient outcomes and reduce potential medication-related issues
- Monitor and analyze population health data and clinical outcomes, identifying trends, and suggesting improvement measures
- Ensure medication safety and effectiveness, adhering to local and federal regulations

Education/Training
- Doctorate of Pharmacy from an accredited institution required
- Completion of a PGY1 Pharmacy Practice Residency from a program accredited by the American Society of Hospital Pharmacist is required
- Completion of a PGY2 in Ambulatory Care Residency from a program accredited by the American Society of Hospital Pharmacist is highly recommended
Licenses/Certification:
Active or eligibility for Massachusetts Pharmacist License

Salary Range: $109,280.04-$171,079.79-
Salary Range Details

A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set, and education. This is not inclusive of the value Sturdy Health’s benefits package (if applicable), which includes among other benefits, healthcare/dental/vision and retirement. For annual salaries this is based on full-time employment.
