This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for an Account Manager, ACOs & Medicare Supplement (MediGap) Health Plans based in United States.
This role sits at the center of high-impact healthcare transformation, managing strategic relationships with ACOs and Medicare Supplement carriers participating in a nationally recognized CMS care model. You will be responsible for building trust across complex healthcare ecosystems that include physicians, care teams, carrier executives, and compliance stakeholders. The focus of the role is not only account management, but driving adoption, enrollment performance, and measurable outcomes for a dementia-care program designed to support families and improve population health. You will operate in a highly collaborative, mission-driven environment where clinical, operational, and commercial priorities intersect. This position requires both strategic thinking and hands-on execution to ensure successful program delivery at scale. Accountabilities:
- Own and manage relationships with assigned ACO and Medicare Supplement (MediGap) carrier partners post-implementation through steady-state operations
- Serve as the primary point of contact across clinical leaders, care teams, physician groups, carrier product teams, and compliance stakeholders
- Drive program performance by monitoring enrollment, referral flow, member engagement, and caregiver activation metrics
- Develop trust and alignment across stakeholders to enable compliant, proactive beneficiary outreach and enrollment workflows
- Partner with internal teams to identify barriers, optimize operational processes, and improve program adoption and outcomes
- Coordinate provider and carrier enablement efforts, including education, training, and communication materials
- Lead recurring business reviews and deliver clear, data-driven performance updates to executive stakeholders
- Support CMS model execution, ensuring adherence to GUIDE Model requirements and partner-specific workflows
Requirements:
- 5+ years of experience in account management, client success, payer relations, provider relations, or healthcare partnerships
- Proven experience working with ACOs, Medicare Advantage plans, Medicare Supplement (MediGap) carriers, or value-based care organizations
- Strong understanding of healthcare operations, population health, and provider workflows
- Ability to manage complex, multi-stakeholder relationships across clinical, operational, and executive audiences
- Excellent communication, presentation, and organizational skills with strong attention to detail and follow-through
- Demonstrated ability to translate data into insights and drive performance conversations with senior stakeholders
- Experience in CMS models (e.g., GUIDE, MSSP, ACO REACH) is strongly preferred
- Familiarity with dementia care, caregiver support, or care management programs is a plus
