Role Scope: 

We are looking for a dynamic, service-oriented Interdisciplinary Team (IDT) Coordinator who is passionate about supporting elderly persons in their journey. Under supervision of the Center Manager, the IDT coordinator ensures that IDT responsibilities are executed efficiently and effectively, including scheduling, meetings/discussions, follow-through. 

Core Responsibilities & Expectations for the Role 

    • Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong. 
    • Continue to raise the bar.  Constructively seek and share feedback and help us implement changes in order to improve clinical outcomes and experience for participants. 
    • Exhibit and honor Habitat’s values. 
    • Provide administrative aid for the IDT including developing meeting agendas, generating reports, conducting data entry, and identifying high priority topics for review during care coordination. 
    • Serve as the facilitator for all care planning meetings and care coordination meetings. 
    • Track all service delivery requests, participant changes in condition, and appeals to support with compliant follow-up. 
    • Coordinate with the IDT to support with compliant care plan and assessments/reassessment completions 
    • Support in ensuring that IDT decisions and care plans aligns with What Matters Most to each participant 
    • Coordinate with IDT to support with timely SDR decisions and notifications to participants. 
    • Confirm that all required documentation and letters are sent to participants in a timely manner and reflected in their chart. 
    • Performs related duties as assigned. 

Required Qualifications:  

  • High School Diploma or equivalency 
  • Minimum of two (2) years of administrative support experience 
  • Experience using Microsoft Word, Excel, and PowerPoint 
  • Previous experience coordinating and facilitating care conference meetings. 
  • Proof of medical clearance for communicable diseases, including a TB test. 
  • Proof of all immunizations are up to date. 
  • Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.. 

 

  • Preferred Qualifications: 
  • A minimum of two years experience working with the frail or elderly. 
  • Previous experience assisting patients/participants with behavioral health and substance abuse issues.

  • Compton, CA (Onsite) 

Compensation:  

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/vision insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, paid time off, and company-paid holidays. The expected salary range for this position is $25.00 -$28.00 hour and is bonus eligible. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
