Job Description Summary:
This position is responsible for providing education and support to metabolic and weight management patients and facilitating communication across the continuum of care. Responsibilities include management/navigation of patients, education and advocacy, and coordination of care with managing physicians, assurance of high-quality care, continuous process improvement, optimal productivity, efficient utilization of resources and customer satisfaction in the Metabolic programs for OhioHealth. Coordination of care will be over an extended period of time and involves multiple modalities of treatment screening referrals, patient communication, direct patient rooming functions, in person patient education, physician communication, following up on order placement and test completion, and a component of data tracking and registry work.Responsibilities And Duties:

Graduate of an accredited school of nursing and currently licensed in the State of Ohio. Bachelor of science in Nursing from a four-year college or university. Licensed to practice as Registered Nurse in Ohio; current Basic Life (BLS) provider Clinical knowledge of surgery processes and weight management issues. Able to establish supportive relationships with a diverse group of patients; exceptional customer service with specialized training in patient care Experience initiatives; detail oriented with exceptional organizational skills Experience as staff RN ( 5 -10 yrs. clinical, bedside nursing in related area of practice).
