LPN, RX Clinical Program Professional

This role focuses on supporting clinical pharmacy programs designed to improve medication adherence, optimize therapy outcomes, and enhance overall patient health. Working in a structured yet patient-centered environment, you will engage with individuals managing multiple prescriptions to ensure safe and effective medication use. The position combines clinical nursing knowledge with outreach and education, including outbound patient communication and medication reviews. You will play a key role in identifying care gaps, promoting Rx education initiatives, and supporting chronic and complex condition management. The work is guided by established clinical standards and procedures, while still allowing autonomy in managing daily priorities. This is a remote role that offers meaningful impact through direct patient engagement and population health support.
Accountabilities
- Support prescription drug optimization and medication therapy management programs to improve patient outcomes, particularly for individuals on multiple medications.
- Conduct outbound calls to patients to confirm medication adherence, provide counseling, and reinforce prescribed treatment plans.
- Participate in comprehensive medication reviews and identify potential gaps in therapy or safety concerns for escalation.
- Deliver Rx education initiatives and targeted outreach campaigns to promote health awareness and medication adherence.
- Document patient interactions accurately within Electronic Health Record (EHR) systems while maintaining compliance with clinical standards and regulations.
- Collaborate with clinical teams to ensure coordinated, patient-centered care and effective program execution.
- Active, unrestricted LPN license is required.
- Minimum of 1 year of experience working as an LPN in a clinical or healthcare setting.
- Experience using Electronic Health Record (EHR) systems for documentation and care coordination.
- Strong communication skills with the ability to provide clear, compassionate patient counseling over the phone.
- Ability to work independently while contributing effectively within a collaborative clinical team environment.
- Strong problem-solving skills and comfort working within structured clinical guidelines and protocols.
